Transforming Lives Through Umoja Health Forum

Aging Better in Camden, Older people in Action (OPA) BAME Partnership

To reduce isolation, all older people from the target group are involved in local activities like walking, sewing, healthy eating, knitting, mental health peer groups, outings, film screenings and theatre. 


Funded by Big Lottery in partnership with 6 UHF partners, Hopscotch/Age UK

The Cultural Advocacy Project

Aims to work in partnership with Community Organizations in Camden to collaboratively set up, facilitate and support initiatives for better mental health and access to other services in the network.

Prostate cancer Project

To carry out prostate cancer health awareness through organising workshops and Open Day to inform, inspire and motivate people about prostate cancer and urge people to get diagnosed early, to benefit from treatment.

Camden Carers Services (CCS)

In partnership with CCS  through outreach and delivering workshops to raise awareness on informal caring in the black communities and encourage people to register with CCS services in order to access and benefit from their services.

Dementia

Aims to raise awareness about Dementia and reduce the stigma by accessing dementia friends training.

Sexual Health , HIV & STI’s

Raising awareness & campaigns on HIV, HIV testingSTI’s prevention, distribution of condoms and making referrals to appropriate agencies. Celebrating annual World AIDS Day by organising joint events..

Sickle Cell Anaemia

Raising awareness on the condition and the treatments available. Sharing experience about day to day coping with living with Sickle Cell Anaemia.

Together we can

Improving employment prospects for volunteers by training disadvantaged members of the community, who are unemployed as volunteers, leading to paid employment.

Community Health Advocates

Part of VAC’s Health Inequalities work to address issues which contribute to poor outcomes for Camden residents. 


Funded by NHS in partnership with Voluntary Action Camden.

PrEp HIV

Organising workshops and carrying outreach work, including encouraging HIV testing for African & Caribbean communities in Camden & Islington. This project finished in September 2024.

Camden Council project Cost of living

We identified individuals struggling due to the high cost of living and made referrals to support services such as Little Village for baby and children's clothes, food banks, and Citizens Advice Bureaus. Additionally, we worked to help secure essential items, such as washing machines and other necessities, to support those in need.

COVID 19

This project provided clear, factual information about COVID-19 to encourage Sub-Saharan African communities to access and receive COVID-19 vaccinations. Running for 8 months in 2022, the initiative focused on making the information easily understandable and accessible to ensure greater uptake of the vaccine within these communities.


Funded by the NHS and Camden Council.

Big Lottery funding

CEPIA Project for 50+ funded by Big Lottery a partnership of 8 organisations namely:

Hopscotch, KCBNA, The BAME Community Action Project (Older People in Action) is made of the following small BAME community organisations/groups: APTO, Cornerstone, EWA, LNM, SSWSD and Women’s Voices, Led by the African Health Forum (now Umoja Health Forum) and the overall project is led by Hopscotch. This partnership had a strong element of capacity building to enable poorly funded BAME community groups to acquire new skills and expertise in a supportive environment so that they can support their communities.

Community Action Research Project

For this project our outreach interventions focus on gathering information about the needs of the communities we serve through one to one face to face and telephone, What's APP, focus groups etc. The insights from this research highlighted the importance of a dedicated link/connecter role, which we were fortunate to secure funding for from The Hampstead Wells and Camden Trust. This role was launched in 2023 and ran for 1 year, proving to be a success. As a result, the project has been funded for a second year, from October 2024.
